Our Services
Empowering your independence through expert coordination and dedicated recovery coaching.
Our Services
At Health Care Partners in Support, we specialize in two core areas: Independent Support Coordination and Psychosocial Recovery Coaching. Because we do not deliver core support services, our only agenda is your success. We act as your objective guide, translating your NDIS plan into practical, real-world supports that align with your unique goals.
Support Coordination
Level 3
Navigating the NDIS can feel like learning a new language. Our Support Coordinators are here to translate your plan into action, ensuring you get the most out of your funded supports.
Our Approach: Instead of just “managing your providers,” we link you with suitable, ethical services while actively building your capacity to coordinate these supports independently in the future. We monitor service delivery closely to ensure ethical boundaries are kept and your funds are utilized effectively.
Psychosocial Recovery Coaching
Recovery is not a straight line, and you do not have to walk it alone. Our Psychosocial Recovery Coaches bring specialized knowledge in mental health to help you build resilience, manage daily challenges, and achieve a meaningful life.
Our Approach: * Strengths-Based: We focus entirely on what you can do, uncovering your personal strengths to drive your recovery goals.
System Navigation: We expertly navigate the complex intersections between the NDIS, mainstream health systems, and housing services to ensure you have a stable foundation.
Trauma-Informed: We are deeply sensitive to complex family circumstances and emotional regulation challenges. We act protectively, ensuring your environment and supports feel safe, respectful, and culturally responsive.
The HPIS Standard of Care

Transparent & Unbiased
We provide completely objective information about providers. Because we are a separate entity from core service providers, our advice is 100% conflict-free.

The "Village" Approach
We collaborate holistically with your clinical teams, allied health professionals, families, and schools to create a unified, wrap-around support network.

Proactive Crisis Prevention
Particularly during life transitions (such as hospital discharges), we closely monitor service delivery to ensure stability and reduce the risk of crisis.
